This contract is made between the Adopter (printed below) and CHAMPS CHANCE INC (CCI).
ADOPTER AGREES TO THE FOLLOWING:
3. If the animal is found to require major treatment, such as an underlying birth defect, the Adopter may return the animal within 30 days of dated executed contract. Adopters must provide Written Veterinarian Confirmation to CCI of the animal's health and the necessity for major treatment.
4. Keep the animal as a pet and companion and not to abandon, trade, give away, sell, nor is the animal to be used or sold for commercial use or research purposes. The animal is never to be turned over to an animal shelter but must be returned to CCI. Violation of this provision of the contract carries a $150 Fine. If the Adopter no longer desires or is unable to keep the adopted animal within the terms of this contract and the animal is returned to the responsibility of CCI, no penalty will be invoked for such action, nor will any refund be given. The adopter will however be responsible for fostering the dog until an adequate housing arrangement can be made with a maximum time frame of 6 months. The adopter agrees to actively assist CCI in finding new adequate housing through participation in activities such as adoption events, meet / greet of potential fosters / adopters and providing updated pictures / descriptions.
5. Provide adequate, high-quality food, water, shelter, exercise, and medical care always and care for the animal in a devoted and humane manner for the remainder of its natural life.
6. Have the animal examined by a licensed veterinarian at least once a year, maintain appropriate vaccinations and other preventive health and dental care, and provide copies of the animal's care records to CCI annually if requested for a minimum of three years and maximum of 5 years.
7. Provide the animal with veterinary recommended Heartworm Preventive and a Flea / Tick Control program for the rest of its natural life.
8. Keep the animal under the Adopter's control and not let the animal roam freely outside of the home unless in a completely solid fenced and secure yard. The use of unsupervised tie-out chains, ropes, dog-runs, pronged collars or similar device is not permitted.
9. The Adopter authorizes any state or local animal care or control agency to release the animal to a CCI representative, hereby designated as Agent for the Adopter, if the animal is found running at large or is brought into a sheltering facility for any reason and the animal's owner is unable to retrieve the animal timely (24 hours).
10. Attendance to at least one full course of obedience training with the dog beginning within 3 months of this contract execution date is highly recommended. No dogs adopted from CCI are to be attack-trained or guard-trained or used in any aspect other than obedience training unless it is under law enforcement contracts.
11. Provide a humane secure collar for the animal to always wear. A custom tag containing the adopters contact information is highly recommended to be attached to the collar in the event the animal is lost.
12. CCI uses implanted microchip identification. If the microchip is not inserted prior to adoption, arrangements must be made with CCI within 2 weeks to have this done. CCI is the registered Guardian on the microchip for the protection of the animal from possible shelter euthanasia if lost or surrendered. The adopters contact information is added to the animal's microchip registry at the time the adoption is finalized. The Adopter will notify CCI of any contact change requests including email, address and/or phone number changes via email (CCIadoptions01@outlook.com) in a timely manner to maintain updated contact information.
13. At any time during the animal's lifetime, if deemed necessary, CCI will be allowed to investigate the premises where the animal is kept and reclaim the animal if Florida Law determines abuse has occurred under the following guidelines. In Florida, the penalties for animal abuse vary depending on the type of abuse. The penalty for animal cruelty might include up to 1 year of jail and a maximum fine of $5,000. However, the presence of aggravating factors might result in up to 5 years of prison and a maximum fine of $10,000. We will follow all the guidelines set forth in Chapter 828 of the Florida State Statutes for Animal Cruelty.
14. Adopter agrees to pay all costs necessary to enforce this agreement including reasonable attorneys' fees, costs of litigation, animal recovery fees, and all medical bills to restore the animal's health back to adoptable status; or a $150 fine, whichever is greater.
ADOPTER AGREES TO THE FOLLOWING OPTION FOR MEDICAL COMPLETION:

CCI makes no implied or expressed warranties regarding this animal other than stated in this contract. The animal is adopted by the Adopter "as is". It is understood by the Adopter that should the animal cause personal injury or property damage after possession is assumed by the Adopter; the CCI, its directors and members shall in no way be liable in whole or in part for that damage. The Adopter hereby assumes all liability for the animal, or any injury or damage that may be caused by it. Adopter hereby waives all claims, losses, or damages which may arise after the delivery of this animal to Adopter, caused by any act or omission by CCI which results in personal injury or property damage to any person by this animal, and the Adopter agrees not to bring any legal action seeking any remedy against CCI in connection with this agreement or in connection with personal injuries or property damage claimed to result from any act or omission by CCI whether by Adopter or any third party.
Medical Records cannot be released prior to receipt of the signed contract indicating plan for medical completion via CCIadoptions01@outlook.com and the Minimum Adoption donation of $350 paid on Champschance.org under the donate tab.
Transfer of ownership rights from Champs Chance Inc. to Adopter does not occur until all criteria is met and contract signed by both parties.
